The three levels of coordination

Your plan sets the level and the hours. If circumstances have changed since it was written — a hospital admission, a housing loss, a carer who can no longer continue — raise it, because that can support a different level at your next review.

• Support connection — short-term help to get started and link with a few services.
• Support coordination — the common level: building your service mix, negotiating agreements, sorting problems out.
• Specialist support coordination — for complex situations involving health, housing, justice or child protection.

Why local knowledge matters in central Victoria

Bendigo has a reasonable spread of providers, but capacity is uneven. Allied health waits in particular vary sharply between clinics, and a coordinator who calls the right three first can save months.

The other regional advantage is knowing when to look outward. For some therapies the practical answer is telehealth into a Melbourne practice, or a clinic in Castlemaine or Echuca with shorter waits. A coordinator who only works from a local list will tell you nothing is available when something is.

What good coordination looks like in the first quarter

In the first three months a coordinator should understand your goals, shortlist providers, make the calls, get service agreements signed and see the first appointments booked.

By the end of that period you should know what is running, what funding is committed and what is still outstanding. If nothing has started after three months, that is a fair reason to change coordinator — you can do that without changing your plan.

Agree at the start how often you will hear from them and in what form. Regular short check-ins beat occasional long meetings, particularly if you live outside the city.

How coordination connects to your other supports

Coordination is funded separately, so using it does not reduce your support work hours.

Coordinators commonly arrange respite and short term accommodation, SIL and medium term accommodation after a hospital stay, plus therapy such as occupational therapy and speech therapy.
