What it is
Frailty support combines exercise physiology, physiotherapy, OT, nutrition, personal care and reliable in-home help — focused on strength, function and preventing decline.
Who it's for
Older Australians who are losing weight, getting weaker, or finding everyday tasks harder; people recovering from hospital; people with multiple chronic conditions.
Home Care Package pathway
Home Care Packages typically fund the mix of therapies, personal care and equipment used to manage frailty.
CHSP pathway
CHSP can fund entry-level supports such as transport, meals, social support and basic in-home care.
NDIS pathway (under 65)
If frailty results from a disability acquired before age 65, NDIS support may be available — including physio, OT and support workers.
