    NDIS Support Coordination in Brisbane

Support coordination is the part of an NDIS plan that turns funding into working supports. If you are looking for a support coordinator in Brisbane, My Care Link Australia is a free matching service — you tell us what you need and we use that as the starting point.

We are not a provider and we do not hold live caseload information, so this page does not say which coordinators currently have capacity. What it does do is explain how coordination works across Greater Brisbane and what to have ready before you enquire.

What a support coordinator does with your plan

A coordinator helps you use your plan rather than delivering the hands-on support. That usually means explaining what each budget can fund, shortlisting providers, setting up service agreements and stepping in when something is not working. The day-to-day work is done by the support workers and therapists they help you connect with.

Coordination is funded at three levels and the level in your plan changes what to expect. Support connection is short-term help to get started. Support coordination is the common middle level. Specialist support coordination is for complex situations where health, housing or justice systems intersect and risk needs active management.

Check your plan for the funded level and the number of hours before you enquire — every coordinator will ask, and knowing the answer saves a round of emails.

    • Explaining your budgets in plain language and what each one can pay for.
    • Shortlisting providers and arranging quotes or service agreements.
    • Coordinating therapy, daily supports and mainstream services such as health and housing.
    • Preparing evidence ahead of a plan reassessment.
    • Resolving problems when a provider is not delivering what was agreed.

    Coordination across Greater Brisbane

Brisbane spreads a long way. A coordinator with strong relationships in the inner north may know very little about providers in Logan, Ipswich, Redlands or the Moreton Bay corridor, and the reverse is equally true. Because a coordinator's practical value is their provider network, the corridor they actually work in matters more than where their office sits.

Travel is a real constraint here. Coordinators can generally claim provider travel to face-to-face meetings, so a coordinator based an hour away either declines the referral or spends plan funds getting to you. If in-person meetings matter, name your suburb in the enquiry. If phone and video contact suit you, say so — it widens the field considerably.

Outside the metro area the same logic applies at a larger scale, and we cover neighbouring markets through pages such as support coordination on the Gold Coast and support coordination in Toowoomba.

How to compare Brisbane support coordinators

Most coordinators read similarly on paper. These questions tend to separate them quickly:

    • How many participants does the coordinator carry, and what is their usual response time?
    • Which parts of Brisbane do they work in each week, and do they meet face to face?
    • Do they have experience with your situation — psychosocial disability, autism, SIL and housing, complex health needs?
    • Are they independent, or part of an organisation that also delivers supports? Ask how they manage that conflict.
    • How do they record hours so the coordination budget lasts the full plan year?

    Registration, plan management and cost

Support coordination is billed against your plan rather than paid out of pocket. NDIA-managed participants generally need an NDIS-registered provider. Plan-managed and self-managed participants can usually work with registered or unregistered coordinators, but confirm this rather than assume it — it changes who you can choose.

Our matching service is free for the person enquiring and there is no obligation to proceed with anyone we introduce. We do not certify registration status or availability; confirm those directly with the coordinator. You can read how provider matching works and our review process for how listings are assessed.

Common questions about support coordination in Brisbane

How do I find a support coordinator in Brisbane?

Tell My Care Link Australia which part of Brisbane you are in, the coordination level funded in your plan and what you need help with. We use that to connect you with coordinators worth speaking to. The service is free and there is no obligation.

Will a Brisbane coordinator travel to Logan or Ipswich?

Many will, but travel time is claimed against your plan and some coordinators limit how far they go for face-to-face meetings. Name your suburb when you enquire so we only put forward coordinators who genuinely work in your corridor.

What is the difference between support coordination and plan management?

A plan manager handles the money — paying invoices and tracking budgets. A support coordinator helps you decide what to spend it on and gets supports running. Many participants have both, funded separately.

Can I change coordinators partway through a plan?

Yes. End the service agreement according to its notice period and choose another coordinator using the same funding. You do not need a new plan to change.
