NDIS change of circumstances — how to request a review
If your situation changes — your support needs increase, you move, or you start a new condition — you can ask the NDIA to reassess your plan. Here's how.
When to request a change
- Your disability or support needs change significantly
- You move to a new state or region
- A new diagnosis affects how you live day-to-day
- Existing funding is running out before your next review
- A crisis or hospital stay changes what you need
How to request a reassessment
Contact the NDIA on 1800 800 110 or talk to your Local Area Coordinator (LAC) or support coordinator. You'll usually fill out a Change of Circumstances form and provide updated evidence.
What evidence helps
- Updated reports from your treating health professionals
- Recent OT functional capacity assessments
- Records of how often you've been using current supports
- Letters from support coordinators or LACs
